Experience:
o 24 months of role experience is required. o 12 months of Home Health experience is required. o 12 months of travel experience is required. o 3 months of experience with Epic is required. Requirements:
o Candidates must have a Massachusetts license in hand at the time of application. o This role will require floating to multiple units o Candidates must live at least 50 miles away from this facility in order to be considered a traveler when applying for this role. o COVID vaccination required for onboarding. Declinations not accepted. o COVID booster required for onboarding. Declinations not accepted. o Flu vaccination required for onboarding. Declinations not accepted. Seasonal Flu Vaccine is required if working during flu season (September 15-April 30th). Must be fully vaccinated (exemptions are reviewed on a very limited basis). MUST HAVE:
1 year Home Care experience 1 year Case Management Experience Must be willing to work weekends/holidays/on call in accordance with department need/rotation 1 reference from last 24 months (Manager/Supervisor reference type)1 reference from last 24 months (Charge/Lead reference type) Proof of identification required Certifications:
